Reaching beyond hospital and clinic walls
We care deeply about the physical, mental and social well-being of our neighbors. No matter where you live or the challenges you face, Cone Health will be right there with you, delivering connected care that extends far beyond hospital and clinic walls.
That’s why we’re forging partnerships that close gaps, reduce disparities and build trust where it matters most – in neighborhoods, schools and community spaces. Through this commitment, we're helping people stay healthy, not just get healthy.
This is value-based care in action – a model focused on prevention, access and collaboration that reaches patients where they are and supports the whole person. It's care that anticipates needs, strengthens communities and changes what health systems can be.
Our community health clinics and Mobile Medicine fleet are just two examples of how Cone Health connects care directly to the people who need it most. Planning for the new Chavis Health Hub in east Greensboro is another.
When you give to Cone Health, you help bring outstanding care into the heart of every community we serve – ensuring access, trust and wellness for all.
You can make a difference.
Help Cone Health bring the kind of programming that will transform community health and serve as a model for others. Below are a few of our most critical funds focused on access. If you're ready to make a gift now, visit our donate page.
School-Based Telehealth Fund
Join our partners in establishing telehealth clinics inside targeted high-poverty schools, where we're making measurable improvements in children's health and reducing absenteeism – all while keeping parents at work and students in class. We've already established clinics in 40 schools across three counties – let's keep going. Join us as we reach more schools and add behavioral health services.
Chavis Health Hub Fund
Cone Health and the City of Greensboro are on a mission to nurture wellness and health equity in east Greensboro. A new Health Hub inside an accessible and trusted neighborhood space, will offer wellness services and preventive programming for residents whose risk is disproportionately high.
Wright Patient Assistance Fund
Meets the special needs of patients not covered by other philanthropy funds, ensuring that no neighbor goes without essential care.
